MomsFirst Program
Program Overview
MomsFirst supports expecting mothers, fathers and babies in Cleveland by promoting infant health and well-being. The program offers home visits, where Community Health Workers provide guidance on prenatal and postnatal care until babies are 18 months old.
MomsFirst connects families with valuable community resources and services, ensures access to health education, and offers group courses focused on topics like child development, nutrition, and maternal care. Additionally, the program advocates for better maternal and child health policies, aiming to create a healthier environment for both mothers and their babies.

Case Management
When you enroll in the MomsFirst program, a Community Health Worker (CHW) will visit your home to understand your family’s needs, connect you with services, and offer ongoing support.
Community Health Workers provide guidance on topics like prenatal care, safe sleep, breastfeeding, family planning, postpartum depression, and more while connecting families to valuable resources.
We also offer specialized case management for teens, individuals who are incarcerated, those in rehabilitation facilities, or those experiencing homelessness.
