  • Notify MomsFirst CDPH Staff by completing and submitting the new hire/onboarding paper work outlined below (also in the Policy and Procedure Manual)

    • CM and CHW:

      • Resume

      • Offer Letter

      • Completed Background Check

      • CHW Certification or CHW Program Registration Date

      • New Hire’s Email

      • New Hire’s Phone Number, if applicable

      • Verification of highest degree completed, if applicable

    • Specialized Social Worker

      • Verification College Degree  

      • Verification of Social Worker License

      • Resume   

      • Offer Letter   

      • Completed Background check   

      • CHW Certification or CHW Program Registration Date *  

      • New Hire's Email   

      • New Hire's phone number, if applicable
